This is a short list of information to have on hand to help make registration easier.
Some of these items are linked to the forms, which can be printed, completed, and brought in when you come in to register your student for the first time.

___11. Name of Previous School, address, City, State Zip, phone number, fax number
___12. A copy of your child's birth certificate (Notification Form)
___13. Written explanation of custody issues, and/or copy of court order (if applicable)
___14. Please let us know if your student(s) require(s) Special Services.(Has an IEP)
___15.Optional Student Insurance Application
